Conference Information
REGISTRATION
Please complete and return the Conference
Registration Form by mail or
fax with your payment. If you are registering on
line, you will need to charge the fee to your
credit card (Visa or MasterCard). If you have
any questions, please contact Natalie Shear
Associates, Conference Coordinators, at 1-800-
833-1354 x 103 or 202-833-4456 x 103 (Voice or
TTY). You may copy the form to handle multiple
registrations. Please provide one form per
attendee. Registration confirmations will be sent
to each individual.
REGISTRATION FEES
Fees include all sessions, materials and meals
unless otherwise noted. The conference fee is
$299 (early bird). After May 1, the fee is $325.
Onsite registration fee is $350.

AFP offers a discounted rate of $199
prior to May 1, $225 after May 1 for self
advocates, family members, direct support
providers and graduate students.
Graduate students must attach proof of
current full time status to obtain the
student rate.

CANCELLATION/NO-SHOW POLICY
Paid registrants who cancel their registration
may send a substitute in their place. Please
contact us prior to the conference if you need
to make a substitution. Refunds, less a $50
processing fee, will be made if cancellation
requests are received in writing before August
22. There will be no refunds after this date.
Refunds will be mailed after the conference
is held.
ACCESSIBLE ACCOMMODATIONS
Assistive listening devices and sign language
interpreters will be provided on request for
all conference sessions. Please indicate your
request on the registration form. Personal
assistants will also be available during
conference activities.
ENVIRONMENTAL/CHEMICAL SENSITIVITIES
For the comfort of those with chemical and
environmental sensitivities, please refrain from
using perfumed grooming products.
